They often live in the margins of society with low or no access to housing, employment,<br \/>\nfinancial resources, social and health care, and face systematic discrimination and criminalisation in<br \/>\nmajority of countries. Many of them have multiple health problems, which can increase the risk of a<br \/>\n(fatal) COVID-19 infection (including long-term diseases such as COPD, HIV, TB, cancer, and other<br \/>\nconditions which reduce the immune system). Harm reduction services are often the one and only<br \/>\ncontact point for PWUDs to access the health service. They provide health and social services as<br \/>\nwell as other basic support, and function as an essential link to other live-saving services.<br \/>\nWe call on local and national governments and international organisations introducing<br \/>\nsafety measures and to:<\/p>\n<p><span style=\"color: #ff0000;\">1. Ensure the continuity and sustainability of harm reduction and other lowthreshold<\/span><br \/>\n<span style=\"color: #ff0000;\">services for PWUDs during the COVID-19 epidemic<\/span>. This includes in particular Opiate<br \/>\nSubstitution Treatment (OST), Heroin Assisted Treatment (HAT), Needle and Syringe Programs<br \/>\n(NSP), naloxone provision, and continued access to Drug Consumption Rooms (DCRs). In addition,<br \/>\nessential basic services need to be provided, including day and night shelter, showers, clothing,<br \/>\nfood, and other services. This is of particular importance to those who experience homelessness<br \/>\nand\/or live on the streets.<\/p>\n<p><span style=\"color: #ff0000;\">2. Provide adequate funding for harm reduction and other low-threshold service<\/span><br \/>\n<span style=\"color: #ff0000;\">providers<\/span>, and supply them with adequate equipment to protect staff and clients from infections<br \/>\n(soap, hand sanitiser, disposable face-masks, tissues etc.).<\/p>\n<p><span style=\"color: #ff0000;\">3. Acknowledge the important and critical role of harm reduction and other low-threshold<\/span><br \/>\n<span style=\"color: #ff0000;\">services in the COVID-19 pandemic<\/span> and address the specific vulnerable situation of<br \/>\nPWUDs and other related groups.<\/p>\n<p><span style=\"color: #ff0000;\">4. Develop specific guidelines and regulations for harm reduction services, with respect<\/span><br \/>\n<span style=\"color: #ff0000;\">to the vulnerable situation of PWUDs and related target groups.<\/span> These guidelines should be developed<br \/>\nin close cooperation with involved staff and the affected communities, and build on international<br \/>\nWHO guidelines, recommendations, and evidence and\/or national COVID-19 regulations.<\/p>\n<p>More specifically we call for the following:<\/p>\n<p><span style=\"color: #ff0000;\">5. OST and HAT should be maintained and take-home regulations should be established<\/span><br \/>\nor extended for patients to have opportunity to come for treatment rarely then ones a week. Access<br \/>\nthrough pharmacies should be ensured.<\/p>\n<p><span style=\"color: #ff0000;\">6. NSP should provide PWUDs with larger amounts of needles, syringes, and other<\/span><br \/>\n<span style=\"color: #ff0000;\">paraphernalia<\/span> to minimise the number of contacts. Special bins for needles and syringes should be<br \/>\nprovided to collect used material at home.<\/p>\n<p><span style=\"color: #ff0000;\">7. Harm reduction services should provide COVID-19 prevention material and information<\/span><br \/>\nfor staff, volunteers, and service users, including soap, alcohol-based hand sanitisers that contain<br \/>\nat least 60% alcohol, tissues, trash baskets, and disposable face-masks (if this is requested by national<br \/>\nregulations), for people who show symptoms like fever, coughing, and sneezing.<\/p>\n<p><span style=\"color: #ff0000;\">8. Drop-in services, day shelters, and DCRs should advise and support PWUDs in<\/span><br \/>\n<span style=\"color: #ff0000;\">preventing COVID-19 infections<\/span>. Visitors should be made to sanitise their hands when entering<br \/>\nand should stay no longer than is absolutely necessary. Kitchens can prepare take-away food to be<br \/>\neaten outdoors. All necessary measures should be made to increase social distancing among visitors\/<br \/>\nstaff with all possible means, and rooms should be ventilated. Overcrowding in harm reductions<br \/>\nservices, shelters, and DCRs should be avoided, by establishing safety measures, e.g. minimising<br \/>\nthe duration of stay, maximum number of visitors, entering only once per day. People with permanent<br \/>\nhousing should be encouraged to stay at home and come only to pick up needles and other<br \/>\nharm reduction paraphernalia and tools.<\/p>\n<p><span style=\"color: #ff0000;\">9. The health situation of PWUDs should be monitored closely.<\/span> If someone shows symptoms,<br \/>\nsuch as fever and coughing, face-masks should be provided and a medical check-up should<br \/>\nbe ensured. Cooperation agreements with public health services, related health units, and hospitals<br \/>\nneed to be established to ensure direct medical support, follow up care, and treatment.<\/p>\n<p><span style=\"color: #ff0000;\">10. Night shelters need to be made available for people experiencing homelessness,<\/span> with a<br \/>\nseparation in place between those who are not infected and those who are infected and need to be<br \/>\nquarantined, but do not need specific medical care and treatment in hospitals. Night shelters have<br \/>\nto comply with the overall safety regulations for COVID-19, and people should not be exposed to<br \/>\nadditional risks for infection through overcrowding and insufficient health care.<\/p>\n<p><span style=\"color: #ff0000;\">11. Group-related services, such as meetings and consultations, should be cancelled and<\/span><br \/>\n<span style=\"color: #ff0000;\">postponed until further notice or organised as online services<\/span>. New treatment admissions should<br \/>\nbe temporarily suspended. Coercive measures (e.g treatment referrals made by court\/prosecutor\/<br \/>\npolice, probation officer visits etc.) should be suspended. Mandatory urine sampling should be abolished.<\/p>\n<p><span style=\"color: #ff0000;\">12. Harm reduction services should establish a safe working environment<\/span> and make sure that staff are well informed and protected against infection. He was the Director of the Drug Policy Program at the Hungarian Civil Liberties Union between 2004 and 2015. He is experienced in working at international drug policy forums such as the Commission on Narcotic Drugs. He was twice elected to the Core Group of the EU Civil Society Forum on Drugs. He is the co-chair of the Eurasian Harm Reduction Network. He has been representing the Hungarian Harm Reduction Network at the government\u2019s drug advisory body in Hungary since 2007. Peter also contributed to building a network of advocacy NGOs in Europe: the European Drug Policy Initiative. He provided technical assistance to several NGOs, and launched several campaigns on drug policy reform. As a member of the Drugreporter video advocacy team, he has produced videos about drug policy issues in a number of countries.
