Why Russia Says No To Methadone?

The HCLU video advocacy team attended the Second Eastern Europe and Central Asia AIDS Conference (EECAAC) in Moscow, 3-5 May, 2008. Our main priority was to interview narcologists (addiction specialist doctors) and create a short advocacy movie on the lack of methadone substitution treatment in Russia. We believe that it is one of the biggest scandal of the world of drug treatment that substituion treatment is banned in a country with 1,5 million people living with HIV – the majority of them infected through the sharing of injecting equipment. Methadone substitution treatment is one of the best available tools of HIV prevention among injecting drug users according to all relevant international organizations (e.g. WHO, UNODC, UNAIDS), the scientific literature proving its positive impacts on public health and security can fill a whole library: it improves the health and social well-being of users, it prevents them from committing crimes, it decreases illicit drug use and needle and syringe sharing among them. Together with the majority of drug treatment specialists from all over the world we are confident that the lack of access to methadone maintenance treatment in Russia is a basic violation of the human rights of people who use drugs.

Russia may rethink on Methadone
This film is a good advocacy material. I strongly believe the concerned persons may visit other neighboring countries that got success in introducing methadone as a substitute drug…
Bangladesh has recently decided to introduce Methadone (Pilot phase) which was one of our long waited dream!

I strongly beleive that in some cases methadone treatment is essential.But we must not forget that methadone treatment doesnot solve the drug abuse problem.If methadone treatment is implanted, the required dose must be given daily under control of staff. In no way methadone must not be given to the drug abuser for weeks in advance. For the simple reason that the drug abuser is unable to contol his intake and or will past methadone to other drug abusers. This means that the market will be filled with legal methadone.
In my oppinion methadone treatment can only be introduced if the necessary control is athand.

Substitution??
You would be better served if you stopped using the term "substitution" to describe methadone treatment. It is NOT replacing one drug with another, but is rather endorphin REPLACEMENT therapy. This is the reason for its efficacy in addiction treatment. Your description only gives inaccurate ammunition to the opposition. From the United Nations on down, this inaccurate description of methadone maintenance therapy only fuels that which you lament.

I've never heard about "endorphin replacement therapy" - all relevant organizations I know use opiate substitution treatment (OST), so it seems there is quite low awareness about the term suggested by you. So before attacking us for using this term maybe you need to make your point to the international organizations and networks, for example IHRA.
Actually I find "endorphin replacement therapy" misleading and inaccurate. Methadone treatment IS replacing an illegal drug with a legal drug, and I don't see the arguments why should it be changed.

Well if you ask me every nation should say no to methadone, but considering this drug is used to help drug addicts. Russia should consider this aspect, there are too many people who need the drug, too many lives at stake and I strongly believe Russia should reconsider this decision.

Many international experts
Many international experts say methadone treatment is critical to controlling the epidemic. Coupled with needle exchange (already in use in some Russian cities), the therapy could largely stop the spread of H.I.V. among injecting drug users.
